Objectives:
At the conclusion of the program the attendees will be able to:
-List the hierarchy of surrogate healthcare decision makers in Ohio
-Delineate the scope of power for any surrogate healthcare decision maker and how it can contrast with the previously stated wishes of the patient
-Identify when a surrogate healthcare decision maker should and should not be making decisions for an individual with dementia.

Social Workers: Hospice of Northwest Ohio (RCS 100003) is an approved provider of Continuing Professional Education by the Ohio Counselor, Social Worker and Marriage and Family Therapist Board. 1.0 CEUs will be awarded IN ETHICS to Licensed Social
Workers and Counselors.

According to the Ohio CSWMFT Board, any late arrivals or early departees will be given certificates accurately reflecting their actual times of attendance. The Ohio CSWMFT Board requires a minimum duration for CEs of 1 clock hour, so attendees present for less than one hour of the presentation will not be given certificates.
